By paying for this service work order I hereby authorize the repair work order to be done with the necessary materials. Pasadena Cyclery and its employees may operate the vehicle for purposes of testing, inspection, or delivery at my risk. An express mechanics lien is acknowledged on the vehicle to secure the amount of repairs thereto. It is also understood that Pasadena Cyclery or its employees will not be held responsible for loss or damage to vehicle or articles left in vehicle in case of fire, theft or any other cause beyond Pasadena Cyclery's control. I understand that there will be a storage fee on $2 per day charged after 7 days of completion. I understand that any item(s) left over 90 days for repair becomes the property of the business.
The above prices, specifications, and conditions are satisfactory and are hereby accepted. You are authorized to do the work as specified.
